Healthy Places Action Tool

The Healthy Places Action Tool (HPAT) was developed and tested for rural Alberta communities to help better understand what influences health where we live, work, play and age. The HPAT can assist in learning community context and inform action. Each tool offers questions related to 5 different settings (Community, Facilities & Organizations, Healthcare, Schools, and Workplaces) that can lead to rich discussions about your community’s strengths and potential areas for improvement.

Remember you are all community experts and hold the wisdom and knowledge of how to create healthier places and social spaces together.
