The Microsoft Word template provided below has pre-populated questions that measure Awareness, Knowledge, Behavioural Intent and Behaviour. Use these as the starting point for your survey. Edit these questions to create a custom survey that suits your initiative and/or add additional questions.
Why measure Awareness, Knowledge, Behavioural Intent and Behaviour?
Assessing participants' awareness evaluates an individual’s understanding of the resources available in their community. By raising awareness, the community is empowered to take proactive steps toward enhancing health and wellbeing.
Knowledge equips individuals with the necessary information to make informed decisions about their health. Evaluating participants' knowledge indicates their likelihood of adopting healthy behaviours.
Measuring participants' behavioural intent serves as predictor of initiative success. Measuring intent offers insights into individuals' motivation to maintain positive health behaviours, which is essential for long-term impact.
Lastly, behavioural change in participants provides valuable insights into the effectiveness of initiatives. By tracking changes in behaviours, such as quitting smoking or adopting a healthier diet, one can gauge the impact of an initiative. By assessing community members' awareness, knowledge, behavioural change, and behavioural intent, we can drive effective and sustainable practices to improve community health and wellbeing. Ongoing evaluation and measurement are key to ensuring continued impact of these initiatives as communities strive for a healthier future.

The goal of the toolkit is to provide users with a structured set of questions. These questions can easily be used in the online survey tool of your choice. This will save time by having all the questions built out beforehand.
