We suggest a survey to understand change in Awareness, Knowledge and Behaviour. 

The Microsoft Word template provided below has pre-populated questions that measure Awareness, Knowledge and Behaviour.  Use these as the starting point for your survey. Edit these questions to create a custom survey that suits your initiative and/or add additional questions. 

Assessing participants' awareness is crucial in ensuring that individuals can access the necessary tools and services to promote health and wellbeing. This helps ensure that the community knows about the various supports and services.

Evaluating participants' knowledge of healthy lifestyles is important. This supports an individual’s understanding of the importance of healthy diets, regular physical activity, and other lifestyle choices that support health and wellbeing.

Measuring behaviour among participants is also essential. Having knowledge of healthy lifestyles and available resources is one piece of the puzzle, however turning that knowledge into action is vital. If specific behaviours can be identified, community-based strategies can be designed to include targeted infrastructure changes and enhancements.

In conclusion, evaluating and measuring participants' awareness, knowledge, and behavioural change are fundamental for initiatives that link infrastructure change and enhancement with health messaging. It ensures individuals have access to resources they need, while also being able to proactively make informed decisions.

The goal of the toolkit is to provide users with a structured set of questions. These questions can easily be used in the online survey tool of your choice. This will save time by having all the questions built out beforehand.
